3. APPOINTMENTS

By scheduling an appointment, you agree to provide accurate and current contact information and to follow any instructions provided by Revival before your appointment.

4. CANCELLATIONS AND NO-SHOWS
Revival Skin Studio requires at least 48 hours' notice for appointment cancellations or rescheduling.
Cancellations or rescheduling requests made with less than 48 hours' notice, as well as missed appointments, may be subject to a cancellation fee in accordance with Revival's current cancellation policy.
Certain services may have additional or different deposit or cancellation requirements. Those requirements will be communicated to you when applicable.
Revival reserves the right to require a deposit or payment to reserve certain appointments.
5. APPOINTMENT DEPOSITS
Certain treatments may require a deposit to reserve an appointment.
For example, Sculptra appointments require a $300 booking deposit, which is applied toward the treatment cost on the day of the appointment.
Deposits may be non-refundable when an appointment is cancelled, rescheduled, or missed within the applicable cancellation period.
If sufficient notice is provided, Revival may allow a deposit to be transferred to a future appointment, subject to the applicable treatment and scheduling policies.
Deposit requirements may vary by service.
6. CONSULTATIONS AND TREATMENT ELIGIBILITY
Some services may require a consultation before treatment.
A consultation does not guarantee that a particular treatment will be recommended or performed.
Our providers reserve the right to determine whether a treatment is appropriate based on an individual's medical history, goals, skin condition, medications, allergies, previous treatments, and other relevant factors.
Patients are responsible for providing complete and accurate information regarding their health history and any medications, supplements, allergies, previous procedures, or other information that may affect treatment.
7. PAYMENTS, PURCHASES, AND PRICING

Revival accepts the payment methods made available to patients at the time of purchase.
